The downloadable PDF forms listed in the Forms Library section are enabled so that you can enter your responses into the each form from your computer. While copies of these forms are also available at the spa., we kindly request that you fill out the required forms before you come in.
Due to COVID-19 safety protocols, we suggest you complete the paperwork prior to your arrival. Otherwise, please plan to come in a few minutes prior to your scheduled appointment time in order complete the paperwork If you are unable to print and need to obtain the required forms from our spa’s guest relations desk.
There are two ways to fill out each form:
1) Â Â Download. Print. Fill Out by Hand.
- Download the Form and print it on your printer.Â
- Fill out the needed information by hand
- Sign and Date each Form
- Bring the signed copy with you to the office
2) Â Â Download. Fill in Your Responses. Sign. Date. Send by email.
- Download the Form(s) to your computerÂ
- Open each Form you have downloaded
- Fill in (type in) your responses (not applicable to all forms)
- Use the Signature Tool to digitally sign each form (Optional
- Enter the Date
- Save the filled out Form on your computer
